Graphics card performs badly (also problem with my monitor)

Hi, I'm a noob when it comes to computers, so, my question to you is, why is my graphics card performing badly? This has been a pain in the ass for me for like 3 years or so, so what I'm asking you to do is give me some advice on what should I do with this card. Every game I play lags on medium setting and not a very high resolution, and even Half-Life 2 lags on everything max. EVERYTHING MAX for Half-Life 2. Like, I ask myself, really, a game that's like more than 10 years old lags on a 2GB graphics card? And that;s pretty much it. Now I know I'll have to give you the specs, because with this explanation, you would get nowhere. So, here they are. Oh, and another thing. During a video or even a video game, my screen sometimes shows these horizontal lines that go from down to up in a continuous speed. Anything you can help me with? I'll provide more info if necessary. Please help!

-Operative System: Windows 7 Home Premium 64-bit
-CPU: Intel Core i5 2310 2.90GHz
-RAM: 8.00GB Dual-Channel DDR3 @ 665MHz (9-9-9-24)
-Motherboard: Foxconn H67M-S/H67M-V/H67 (CPU 1) (the manual says it's a H67M series MB)
-Monitor/s: Philips 200WS (1680x1050@60Hz) (used regularly for gaming, browsing , etc.)
SONY TV (1680x1050@60Hz) (used only for movies, etc.)
-Graphics card: 2047MB NVIDIA GeForce GT 630
-Storage: 931GB Hitachi HDS721010DLE630 ATA Device
-Optical Drives: ATAPI iHAS122 ATA Device
-Audio: Realtek High Definition Audio

As said above the lines your seeing is most likely a card going bad if your overclocking it.. There are far better cards out there now even a 750 ti would be better then what you have example a 960 would be a good fit for your system as well its probably time to upgrade to a better card now that way it will enable better game play then you are getting at this time because a lot of the games now are requiring so much more then what you have... as a GPU
horizontal lines are most of the time an indication of bad drivers or your card going broken. try updating the drivers if that does not solve it you might want to reconsider buying a new one.
