Graphics Cards Issue?- My Computer has Horrible Texture Glitches/Artifacts

I have a Dell XPS 8700 with an Intel Core i7-4790 CPU, 16 GB of RAM, and an NVIDIA GTX 745 (I know the GPU is lacking and I have considered putting in a GTX 970).

I got the PC about two years ago and for 8 months it ran smoothly with no texture glitches nor problems with gaming. But since then, there have been checkered green squares popping up on my display and horrible artifacts in games. Last time I ran Rust and Overwatch both at the lowest possible resolution, they either crashed or were unable to play.

Things I have tried already:
Uninstalled/Reinstalled NVIDIA GPU drivers
Cleaned the card and blew dust (I honestly have no idea what the problem is)

Run MSI Afterburner while you play, to see what kind of temperatures you are getting. Are these artifacts only showing up in gaming, or even when you're just on your desktop? If it's all the time then I think your videocard is done. Run on the integrated graphics to see if your computer works without the videocard.

Cleaning off the videocard is kind of pointless if you don't monitor the temperatures before and after. How would you know if it's running cooler or not?

I run a program called "Speccy" that does relatively the same thing.
CPU goes up to 45 and GPU goes up to 68.
I ran my computer on the integrated graphics. It works without glitches but has trouble running games of course haha.
I think its time to change the GPU
