GTX 970 core clock locked at 405mhz?

So i was just sitting there playing forza horizon 3 when my fps just shot down immediately, i checked everything then found out that whenever i play any game my gpu clock speed never goes higher than 405mhz which is clearly the source of the fps drop, how do i fix this and what options do i have? I also wasnt oc'ing at the time this happened or messed with any graphical setting so i dont see why this could have happened
 


so basically yes, i just tried to load into a different part of the game and when i loaded back in i suddenly was down to 17 fps, i hadnt changed any software or hardware at all id just been playing the game for probably 3 hours until this happened.

my gpu right now is chilling at 45c even when playing the game and still the clock speed wont rise

im running a fx-6300 cpu, corsair rm750i psu, and a gtx 970 gigabyte xtreme with some corsair memory

 


and just as im waiting for a response to this, i was loading up a youtube video when suddenly my screen froze and recovered and when i look back at the reliablility monitor it states i had a hardware error and i didnt realize this but everytime my forza would crash in the around 3 hour session i would have a hardware error, which i assume was my graphics card
