Has my SSD failed? Or what could be the issue?

I have a feeling my SSD has died over night, my mobo is picking it up in the bios but when i try to boot the OS from it, it gets past the BIOS then when its supposed to load OS it just goes black and nothing happens. I've checked all memory, my GPU's used both of their hdmi slots and my iGPU hdmi slot, checked multiple monitors.

When i try to boot from my other hdd's it attempts to load an OS but says 'boot mgr not found bla bla'

So surely this means my SSD has failed?? Just want to clarify.
Downloading Windows 10 right now to install on another HDD to check.

Specs:
i7 4790k 4.6ghz
4x8gb (32gb) ddr3 1883mhz ram g.skill ripjaws
2xgtx 970 sli
z97 msi gaming 7

ssd: OCZ Radeon R7 120gb (i know its crap lol)
sshd: forgot model cba checking (1tb)
hdd: old toshiba 1tb hdd

change boot order to match the iso you downloading now (ie, usb or dvd)
boot from it and on second screen after language choices, select repair this pc, not install
select troubleshoot
select advanced
select start up repair - this scan hdd and fixes anything it can - will need logon details

if that doesn't work, boot from iso again and instead of choosing troubleshoot, select reset - you get choice of keeping files and settings, or wipe it all. I would keep files...
after you choose, windows will save what it needs and reboot and reinstall win 10

its more likely a windows problem, not the ssd :)

i already had done most of it, it's stuck in a busy state, the bios recognises it but diskpart can't find it, it's still in warranty i've contacted OCZ, in meantime i bought myself a samsung 850 evo, if they replace my r7 i'll just stick in my precision t3500, thanks for your help tho :)
