Haswell OC Can't pass 4.2GHZ?

I can't seem to pass the 4.2ghz mark. No matter what I do or how much voltage I pump into it. I can't get passed 4.2ghz. I don't know if I'm doing something wrong or if I just got a bad chip. I want to get between 4.4/4.6ghz. Even higher would just be icing on the top.

My i5 is running 4.2ghz@1.25 stable.

CPU: i5 4670K
Cooler: Corsair H80i
Motherboard: MSI Z87-G45 Gaming

It might be the chip. I have the same chip/mobo and a lower end cooler. I'm pretty much stuck at 4.2ghz as well.

At 4.2ghz (dynamic mode) I'm stable at 1.230 volts (adaptive mode). Thats with leaving ring/cache ratio to its default at 38 and with XMP enabled. If I increased ring ratio any higher to try to get it closer to a 1:1 ratio to my cpu I loose stability. My temps are fine rarely go over 70C under load.

I also can't even get 4.3ghz to be stable and thats with nearly pushing voltage to 1.300 which was my max i'd be willing to go for a 24/7 overclock.
