HDMI just randomly stopped working

So about two days ago I was just doing my normal browsing on reddit when my HDMI monitor just flickered out of no where then after it was dead my display started to act up. Whenever I put the HDMI cord into the screen it pulls up what looks like the CTRL ALT DEL menu, and most of the screen is black except the bottom right corner. The monitor I am using is working fine with HDMI as I was just playing my ps4 on it. My GPU is a 750 ti.

I am using a DVI to VGA adapter for my second monitor and it works just fine.

OK... then the video card is OK..

Got a different cable?

Is there a diag. for the nvidia to see if that port has gone?

what ports are on each monitor & videocard? is not a monitor issue as that hdmi monitor works ok on ps4.
