Just got a Samsung Galaxy S5 and transferring the 64GB Sandisk micro sd from my Galaxy S4. Here are the steps I performed:
1. First, I unmounted the card safely from the S4.
2. Put it in a Sandisk brand card reader (SDHC) - I believe this is the one the card actually shipped with
3. Using Windows explorer, I right-clicked and chose Quick Format with options exFAT and 128 KB allocation unit size. It shows capacity as 59.4 GB
4. I let it run for about an hour and it never completed the job (also the process never stopped responding, which is what I expected to happen)
5. So I had to just pull the card without doing a safe-remove, which probably was not good
6. Tried just putting it in my S5 to use it's formatting function but it fails every time (tried 10 times now). It locks up the phone during the erase function and then it reboots.
