High RAM Usage

So, for a couple of weeks, my computer has been acting very slow and games are all of a sudden stuttering like crazy (GTA5 and the Division to name a few) I don't exactly know whats causing the problem but i'm thinking it's my memory, when i'm playing GTA and have a few extra programs like steam and discord open, it's at 95% and I have 12Gb of DDR3. From what I know there was a problem with peoples page files but it doesn't seem to be the problem with me, I changed it a few times and it didn't do much. I've already tried doing everything from disabling Runtime Broker Disabling Superfetch and Changing things around in Regedit, nothing has fully worked and i'm just looking for a answer so I can go back to how my PC used to run. Thanks to anyone who helps.

In task manager click the processes tab and then click memory to sort by process. Paste that screenshot on the forum.

Even with google closed ,it still lags quite a bit, so that doesn't seem to be the problem

just close what u dont need in game...i doubt u need steam (store) opened

On the performance tab of task manager. What is your system up time?

Just under 4 hours right now, but even when it's lower, it still is slow.

Sorry for the late reply, I have figured out it's because of my disk, it's old and it's not working properly, i'm currently trying to upgrade my disk, memory and motherboard, they're all very old and if I find a fix now, it's not gonna last long, but thanks for trying to help.
