How did my usb get this?

memory test was recommended, so I made my 16gb usb flash bootable with memtest86, now drive's size has reduced to 48mb. Formatted several times but still no restoration. I am surprised how can memtest86 still booting itself even when empty usb. I just want my flash back to 16gb.
Experts! please ( i also tried formatting from disk management)

1. download ccleaner
2. once the program is launched go to tools
3. in tools there is a thing called drivewiper (ik it may sound scary but its going to clean it off completely")
4. where it says "wipe free space" change that to wipe drive
5. after wiping is finished let windows reformat it
