I want to be sure if my GTX 660 is failing

After the power cut I couldn't find sleep option. when I open Geforce Experience a window that shows up in the bottom right corner telling that there is a new game ready driver came every time I install the nvidia driver while I have already installed the latest driver.
And screen resolution stay at low after driver has installed so I manually change it to high.
When I open videos in YouTube screen is divided into a horizontal lines like when a game runs with too much fps.
and the screen is vertically divided into parts I mean there is a normal/sharp vertical part and beside it a misty/unclear part then a clear/normal part and so on but u have to be very observant to notice that . I have been using my card for 3 years without problems so its dusty but I don't think thee heat is the problem.

Is installing the Nvidia driver over windows update driver cause any thing wrong ?

My question is this a software (driver) issue or its a hardware issue ?
Running windows 8.1 64 bit with Galaxy GTX660, 4gb ram and I5 4th on ASrock b85m
 
I had similar issues with my two 660 Ti's. I cant guarantee it is the same problem, but mine was due to a dust buildup in the fan and exhaust ports. I recommend taking the card out and cleaning the dust thoroughly. It was pushing my temp to low-mid 90 Celsius and was shutting down when I stressed it in games. Hope this helps!
If you are not sure if it is overheating or not i recommend running HWMonitor (free), and having it log your file. Meaning that next time you are playing and it shuts down or reboots, you can check that log and see the temp and all at the time of the shutdown.
 
I was mean by the power of the whole street cut off not just my computer so I don't think its a temp issue however I didn't monitor it but I can't hear the fans getting noisy
 
It wouldn't hurt to monitor it anyway just IN CASE that is the problem. You will at least have a record of what the card is doing when it forces a restart.
If the problem persists, perhaps fully uninstall Geforce experience and the video drivers, and do a fresh install.
 


Are you uninstalling the existing drivers or just installing on top of them?
If you are not uninstalling first, try that.
 
Try moving your card to the lower PCIe slot. If it is still giving you this weird video output, then my guess would be the card IS dying.
