Do all four wires (red, green, black, and yellow) on the phone jack need to be connected for the DSL modem to work correctly? I keep loosing my internet connection and some times I can't get on line at all. The message tells me that there is a break in or at my access point. I am instructed to reboot Modem/router and try again. no luck. The phone guy has checked my interior lines and has passed them and my server has sent me a new box but still the problem persists. I have a tablet that will sometimes get online and then not, so I concluded that it is the server and not my PC.

Solution
If the dsl connects but keeps dropping, it a line issue.
When you restart your modem the DSL led should flash and then stay solid, that is the DSL service syncing.
Once it goes solid the DSL has synced and the Internet led should start blinking. That means you have a connection to the internet.
If the DSL led goes off at times and then starts syncing again there is a problem with your line.

In this case you should contact your ISP for assistance.

The reason this usually happens is that you have a bad quality connection.
In most cases the ISP can set your line to sync at a lower frequency (speed) so that the line can be stable.
What kind of connection do you have?
