Internet survey

Doing a little survey for my Marketing 3200 class. My group came up with the following survey and I would appreciate some diversity... I mostly have college kids answering (but still answer if you are in college age).

Thanks! I thought about posting it in a more active forum, but I wasn't sure... Most sites that I've been on would have frowned and moved it to the college forum. Haha.

Its a bit longer, but I'm doing it for a psych research. Chronic illnesses. w00t!

One suggestion - rename the "56 and above" age group to "56 to dead"
