Question Is my GPU dead?

Past two nights my screens have just been going black, audio then cuts out but everything inside my PC is still running.
Today I noticed some white flickering lines before it dies.

Here are the specs and my PSU is Corsair RM650X.

I've had this system built for just over a year but the GPU is a few years older than this build as I grabbed it from my old one.
