is my graphics card failing?

my issue is every time i restart my computer it will load to the windows logo then the screen will say dvi disconnected and go black screen i can hear the computer still fully start up just fine behind it and when i enter safe mode i can delete the display driver and restart my computer and most of the time that fix's the issue until i restart the computer again then i have to go thru that same issue again i happen to have a friend who has the same graphics card rx 580 and it happens with his as well currently have it working on his graphics card so its what im using since he just got a 1080 i have reformatted my computer and the issue has persisted through a reformat so im unsure if its my motherboard or graphics card thats failing or even if my hdd is failing

ryzen 1600x
corsair vengence 16gb of ram
rx 580 nitro + 8gb "his is a power cooler"
10000rpm velociraptor hdd and a 500gb sandisk ssd
asrock ab350 pro4 motherboard
 
Yes, it looks like your gpu is dying. What psu is installed in your pc? Can you also try to use a different output of your videocard and a different input of your monitor? How old is your monitor, do you have another one to check? Do you have another vga to check (your buddy's for example). But first tell us what psu you use.
 
its a rosewill 650w arc series..... i disabled fast start up after talking to someone from amd and switched out his graphics card for mine after i did and my computer booted just fine im not sure if that fixed it but i didnt have to go into safe mode to delete the driver again or anything so im not sure why that would be the issue after using this set up for a year only to start having a issue that started 3 weeks ago thats fixed by that
 
Updates have been heartache. I got a build i got for my boy running an i53470 just got it running a bit better especially since i pulled a Nvidia gtx 660 the nvidia drivers been horrible. Phased like 3 Nvidia cards out in favor of the AMD cards.Just threw a Good nice Gigiabyte Windforce R9270OC 2G with 18.4.1 drivers stunned me it running really impressively. Nvidia drivers have been holding there cards up all this year where the AMD cards the drivers havnt been that bad just idiots who dont install them properly just the issue. Something messed up with the 10 & other weird bugs fallen into the 10 subspace & plagued some builds, some left unaffected.
 
im still scared to turn my monitor off wile what the amd guy may have fixed the issue with shutting down and having to delete the driver when i turned my monitor off before it would give me the usb disconnect sound about 5 mins later and when i turn my monitor on it would say no dvi signal so i have a screen saver atm running like i have for the last 3 weeks
