Question Is my SSD dead?

I recently unplugged my PC to clean it and now it freezes on the bios Splash screen. If i unplug my ssd I get the no boot device error but I am able to get in the bios. Below is the crystal disk info.

Here are my specs. Everything was working fine before I unplugged it.

CPU: i5 2500K
Motherboard: Asus P8Z68-V Pro
Ram: Corsair Vengeance DDR3 1600 (4x4GB)
SSD: Crucial M4 128GB
HD:WD Black 1 TB
PSU: Corsair HX850
Graphic Card: GTX570
OS: Windows 7

Yes, I've already tried all of them and still freeze at bios splash page.
