[SOLVED] Is this image a GPU related issue?

Well it might be, but it may also be that somrthing is wrong with the TV or the display cable, have you tried connecting a different device to the TV port? (hdmi I assume)
Have you tried using a different hdmi port on your TV?
Have you tried using a different hdmi cable

If so try:
1) Clean install of the driver using DDU
2)disconnect the gpu from the pc and reconnect
3)at this point it might be a bad port, try using a different port on the gpu (you can use DP to hdmi adapter)
