Issues with HDD

So, recently my PC suddenly started to freeze up causing me to reboot it, once rebooted it says "Please insert boot device" so Im guessing there is an issue in detecting the HDD, Or it;s dying. I noticed my download rate on steam had a graph of both network and disk activity, when downloading my HDD line will begin to sink and go to 0, causing my PC to again freeze. I decided to order an SSD and completely unplug everything and reseat it all. I went on to my HDD activity graph and say occasional dips, this lasted for only a split seconds where "disk active time" would drop to ~50%, is this bad or just the disk not doing anything because it doesn't need to? I have tried "chkdsk /x /r /f" and numerous others, my hardrive however will work when I unplug it and re plug it, but the freezing issue again arises when I log on and do some sort of read-write process. Anyone know of any good tools or commands that will keep a good check of my HDD? One I use says my HDD health is good, so im guessing it might be a wiring issue?
 
Solution
Usually your best bet is to visit the website for the manufacturer (Seagate, WD, Samsung, etc) and get their diagnostic software, and test the drive with that. Overall it sounds like the drive is dying, but it could still be some other issue.

Edit: Also, if you haven't already, try a new data cable with the drive.
