KVM switch recommendations (HDMI 4K @ 60Hz, USB)

I'm looking for recommendations for a KVM switch with the following capabilities/options.

2+ ports
HDMI 2.0
4k @ 60Hz
USB (2/3.0)
Under $200 US

Can be bus powered

Would prefer when changing ports, the switch keeps the connection alive with the other computer so it won't re-adjust window sizes/resolutions.

Hello... reading up on the KVM & KVMP types of devices here... https://en.wikipedia.org/wiki/KVM_switch

It appears too "maintain" connections to other computers you need to use a "DDM class" models.
