[SOLVED] Monitor shows nothing, randomly

Hey guys!
My friend has a very weird issue with his PC.
While the PC is open (doesn't matter what programs are running, happens very randomly, although she noticed this mainly, if not only, happens in the game League of Legends, but as it happens randomly and not quite frequent, we can't say for sure if it's the game's fault), maybe once every 4-5 hours, the monitor just displays no image. It's working though, you can power it on and off, the welcome screen shows up, but it just loses connectivity with the PC, it doesn't display things.
Restarting the PC fixes this issue but it's very annoying, today happened twice. Sounds still works and everything can be controlled (for example while gaming, he can still control his in-game character, click things etc)
He has a Samsung TV 1366x768 (not Smart TV, used as a monitor, connected through HDMI). Computer doesn't see the TV model, sees it as "Generic PnP Monitor"
Any ideas?
Could this be a GPU issue? Driver issue? (all his drivers are up to date)

Start off by accessing the power settings in control panel.

Once its open, what does it say, if her tv/display is set to sleep after 1 - 4 hours.

Toggle it to never go to sleep, also same thing for the hard drive which is "hibernation mode".
What is her video card brand name and model number by chance ?
