Question No Output - Random

I built my PC about 5 years ago. For the past 2-3 years, I have this issue where my monitor goes to sleep due to no output while performing a random action. It could be clicking a button or clicking in a textbox or clicking in the browser address bar. My screen freezes and then goes to sleep. My PC is running but no amount of keyboard or mouse activity would bring the screen back. Restarting my PC is the only way to get the screen output back.
This happens randomly a few times a week and is quite annoying. I would like to know if anyone else has seen similar behavior and if yes how did they resolve it. This behavior has survived a few OS reinstalls.

My PC:
Windows 10/Ryzen 5 1600x/32GB Memory/Asus RX580/Corsair AX760

What do you have your Power Options set for sleep. Computer should catch your using it and not put monitor to sleep. I have had to set never sleep for systems with your same issue. Monitor goes into sleep and your correct reboot only way to move forward.

I have screen set to turn off in 15-mins and for PC to never go to sleep. However, this happens while I am actively working on my PC.
