Question No signal on monitor in middle of gaming or after i alt tab or exit game

Hi guys,
I had same problem couple months ago that in middle of gaming or when i exit or alt tab game message no signal pop up on monitor and i hear some crash sound. After that all i can do is restart pc on button. I changed in display settings display to 60 hz it was on 59 and that fixed it. But yestrday it started again. I have newest drivers, i tryed to switch ram position, tryed other monitor and cable but its same. And yes i forgot when i rs pc 2,3 sec fans on gpu wokr on 100%.
My pc specs are:
GPU: RX 580 sapphire nitro + 8gb
CPU: amd ryzen 5 2600
MB: Gigabayte AB350-gaming
PSU: Cooler master lite 700w\
RAM: 2x kingston hyper x fury 8gb 2666 mhz

Hello are you using a dvi to vga adapter or something like that??

yes ,im using hdmi to vga cable but i also tryed direct hdmi cable on other monitor and same thing happens after 15,20 mins of gaming message pops out. Its not every time after 15,20 mins but its very often i instaled earlyer drivers and now its only when in alttab or exit game
