[SOLVED] ntoskrnl.exe blamed for various BSODs

I built a pc and since day 0 it has had some BSODs. I reinstalled Windows 10 a handful of times and it persisted, until one time, it kinda fixed itself... It would still face a BSOD about 2 or 3 times a month, but it was manageable. Until this week that I was to play some csgo, and was facing some lag, and noticed that I had not installed the AMD Radeon software. After I installed it, the BSOD came back. I uninstalled the software, and it continues. I uninstalled from windows' Add or Remove Programs APP, reinstalled and uninstalled it from the Display Driver Uninstaller v18.0.4.2 program and it still persists.
Note that BSODs DID happen without the Radeon Software, just much less often.

My setup is:
Motherboard: Gigabyte GA-AB350M-DS3H V2
Power Supply: Fonte Gigabyte GP-P550B 550W, 80 Plus Bronze
CPU: AMD Ryzen 7 3700X 32MB 3.6GHz (4.4GHz Max Turbo) AM4
GPU: Sapphire Pulse AMD Radeon RX 5500 XT, 4GB, GDDR6
Memory: HyperX Fury, 16GB, 2666MHz, DDR4, CL16 (2, dual channel)
Storage: SSD WD Black SN750 Heatsink, 500GB, M.2 NVMe, R:3470MB/s, W:2600MB/s
Display: Monitor Warrior LED 23.6´ Full HD, HDMI/DisplayPort, FreeSync, 144Hz 1ms - MN101 (Using: DisplayPort, 144hz, 1920x1080)
OS: Windows 10 Pro 20H2 (build 19042.1110)

  • No 3rd party antiviruses
  • No overclocking (never did, but did reset bios settings just in case)

The most common (but not only) BSOD errors I’m getting are:
  • APC_INDEX_MISMATCH
  • IRQL_NOT_LESS_OR_EQUAL
  • DRIVER_IRQL_NOT_LESS_OR_EQUAL
  • KERNEL_SECURITY_CHECK_FAILURE

Blamed Drivers:
  • Ntoskrnl.exe
  • Win32kbase.sys

Blamed Filenames:
%WinDir%\system32\win32kfull.sys
%WinDir%\system32\win32k.sys
%WinDir%\system32\PSHED.dll
%WinDir%\system32\drivers/afunix.sys

Some triggers I noticed:
  • When I alt+tab
  • While watching a video (on youtube the video image would freeze in front of the blue screen happening)
  • When opening a livestream on discord
It does not happen ALWAYS I do these things, but they happened with synchronicity at least once.

Somethings I already tried:
  • sfc /scannow - fixed some things, but the problem persisted.
  • chkdsk /scan - no problems
  • memtest (extended, 6 steps) - no problems
  • dism /online /ScanHealth
  • dism /online /RestoreHealth
  • Disabled virtual memory
  • Updated every single driver
  • Downgraded some drivers
  • Installed Realtek LAN and Audio drivers
  • Updated BIOS (F50 to F51)
  • Installed Optional Windows updates
  • Uninstalled recent already installed Updates
  • Tried using only one RAM stick, tried the other one, and changed channels from 0+2 to 1+3

So, I entered the bios again, and while was madly angry at this PC, I was randomly changing every setting I could think of in the bios. Some USB support for a every kind of combination of letters I never heard about, also I entered some psu settings and changed enable to disable, others from disable to enable.
Bottom of line is, I don't know what I changed there, probably a lot of things, but it somehow worked (at least it seems). I spent the whole night watching discord streams and playing games with youtube running in background. Also I left a video playing during sleep and when I woke the pc was safe and sound, the way I left it.

I wish I could say what exactly was that I changed and made it work, but I changed so much things at...

So I reinstalled the SO, formatting the SSD and installed AMD software + chrome, and now when I play a video, after about 2 hours the screen goes black, but without any bsod. Its not like a black screen, its more like the image stops going to the monitor. (this times I was watching fullscreen)

And just as I was writing this, a bsod happened.
(here I tried to watch w/o fullscreen to see if the screen would go black too)

I played another video back in fullscreen and the result is the same: black screen but no bsod (and the pc do crashes, the audio stops and pc is not responsive)

And I just discovered that if I have my foot touching the ground, with a toe on the wall and I touch the mb backplate (that metal thing that protects the input's arounds) It passes energy through me. can it have any relation to the problem?

You shouldn't be getting power through the backplate. that makes me wonder about wiring in PC now.

Might want to get PC looked at by someone else, since you made it. They might spot the problem. Can check if its really GPU or something else causing the errors. repair shop springs to mind. 2nd opinion if nothing else.

Gigabyte have some problem models with regards to PSU but I don't think its yours exactly


They aren't the best brand for PSU anyway. video shows that the same model can have different parts in it, and if they doing that with their more powerful models, who knows about less powerful cheaper ones. I would get a PSU by Corsair or Seasonic, they won't damage your parts while you use them.

If PSU lacks power, or can't maintain the power the system needs, it can cause black screens as often GPU is the thing that shows the effects of power problems first, next is USB will stop working. GPU can have most power draw in systems after all.

I never suspected that bc i did a stress test once, not for long dough, about 10 to 20 minutes... But now that you mentioned, I have a external hdd that a couple minutes after a data transfer it will like freeze, resume, freeze, resume (i mean in kbps). stay in 0kbps for a few seconds and then transfer for more 3 seconds and then go to 0kpbs again. I always thought it was the hdd, since its very old (16GB of total space, so you can see how old we are talking about)

also my mouse sometimes will shutdown, and wont work w/o a restart even if plug in another port (don't know if this is somehow retaled)

very old... my first hdd ever had in a pc was 30gb, and thats 21 years ago... so how old?
Its age could be reason for its behaviour but you never know.

I still think you should get a repair store to look at PC for you. If the problems exist through a few installs its likely hardware. Drivers aren't so persistent.
tell them about the backplate.

Heey, so here I am...
Turns out that they spent two days trying to get a bsod and it never happened... I think it is safe to assume that everything inside the pc is ok.
That leaves us with little yet to try:
  • House Electricity :: I tryied a couple of different power plugs in diferent rooms, so, if that come to be the problem, it is all over the building.
  • Headphone :: Tried without pluging it and got a bsod.
  • My PS/2 keyboard :: yet to be tested
  • My monitor :: I'm using the DisplayPort in 144hz, 1080p full HD. Yet to be tested: Never tried using hdmi, could it make a difference?
  • And finnaly, my USB mouse. This one is my most suspicious thing. I tried using another mouse and got a bsod, but I'm now using no mouse connected to the pc for about 2 and a half hours now, and got no bsod yet. What could it be? I cannot imagine that its the mouse's fault... maybe some usb driver? I don't really know anything about that.

I also tried to use the monitor with HDMI and it took a couple hours but I got a bsod too.

[edit] I also tried to use monitor at 60hz and nothing changed

[edit2] Tried without the keyboard, took a long time but got a bsod too

I don't think HDMI/DP would cause a BSOD.

So which USB slots? USB drivers are contained in Windows, and I don't see any drivers installed on your PC that would cause USB errors.
You get a BSOD with any mouse or Keyboard...

its only going to have AMD USB on it. Some older boards have 2 different chips for USB but not yours.

Do you have the dump files from these crashes?

I mean, I was just reinstalling windows and i god a bsod during the instalation process (when you select region and keyboard layout)

So you tried to clean install? At that stage of installation, only things being used is RAM/CPU/Motherboard & PSU
it doesn't touch drives until you show it where to install too.
