Question Nvidia alt+r monitoring feature, HWiNFO ?

When i use this feature my GPU usage is almost always at 0%-15%, but sometimes it spikes to 20-30 and rarely 40 percent and just stays that way for some time. Sometimes, when i'm on my desktop with no other apps open it just stays at 30%. the alt+r updates more frequently than HWinfo, but they seem to agree with each other. I've used Dr. Web and it showed no problems I have multiple questions: - why do these apps and task manager don't display the same info? Task manager often shows that my gpu doesn't go beyong 3-6 percent idle (on the 3D tab) - when i'm on my desktop, does the GPU continue to render the same picture continiously? Because monitoring apps keep showing increasing usage when i'm on my desktop with no apps open - why can't all of this be done with the help of my CPU's integrated graphics?
