Packet Loss and random Disconnect/ Reconnect

I'm experiencing packet loss on my internet connection. The connection also disconnects and reconnects for a split second at times. Whenever I ping google.com or 8.8.8.8, I have 25%, 50% or 75% packet loss.

I'm on a desktop, connected to a modem through ethernet cable. The modem is connected to a router for wireless networking.

First off figure out where the packet loss begins. Inside your network out outside. You can do this by pinging each hop till you find where it starts. Then contact the appropriate company and have them fix their stuff.

I'm sorry, I'm not very knowledgeable on networking. How do I ping "each hop"?

find out the IP address of your router/switch and ping it. You can then do a traceroute to say google.com and ping each hop till you see where the packet loss starts.

You have packet loss right inside your network. I would first try swapping out your ethernet cable from the computer to the modem. I would then try figuring out what your signal strength is for the wireless portion as I never trust wireless connections.
