PC Crashes Under Load (GPU)

I can play games like League of Legends, Even browse the web and watch videos fine. But when I play games like CS:GO I can get around 10 minutes in before I freeze and sometimes lose signal to the monitor. And when I try to run Furmark it does the same thing. When I switched to a super old card it worked, but had to turn down graphics. It seems like it's a GPU issue (prime95 is passing just fine)

CPU:i5 3570k
GPU: Gigabyte GTX 950
RAM: 2x4gb Kingston
HDD: 1TB WD Blue
PSU: 600W EVGA 80+

Things I've Tried:
Upgrading PSU 750W EVGA Gold Cert
Upgrading to GTX 950, old one was 660 and it was having problems as well.
Fresh install of windows on different HDD
Prime95 - passes 2 hrs of tests (longer than CS:GO)
MemTest
1 RAM stick at a time method
Switching to old motherboard
2nd PCI Slot
