Question PCI-E USB hub card burnt taking everything with it. What went wrong?

So I replaced a motherboard and everything seemed fine. A burnt smell came from the case some time later and I turned it off. It was my inateck pci-e to usb 3.0 card that burnt. Worse, it took down all peripherals attached to it - keyboard, mouse, etc. All gone.

What could have caused it? It was installed the same way in the previous motherboard (including the power from the PSU).

Now we're replacing peripherals and I'm a bit scared of using that slot again. Computer is working fine, though.

I can't see a pci card taking out connected devices unless they were overvoltaged. I do see a note below to insure the wire colors match when connected.

It's disconnected now but that might be it. Ugh. What a dumb mistake.

On second inspection, that may not be it. I didn't use that 4-pin connector. It was an SATA power connector directly from the PSU. Interesting...
