Please Help Me

hey guys
i have a problem i have the GD Hardware Scan program
and the program saying that i have this video card installed:

but the video card that i have is this one:

and every time when i open a game my screen goes black and saying

VGA NO SIGNAL

and i dont know why
i got a new power supply
i changed the cables to my power supply and my monitor
and i get the no signal message only when i open a game
and then i have to restart my computer manually
PLEASE HELP.

Graphics Card:GeForce GTX 950 MSI Gaming 2GB Edition
Processor:Cor i3-4160 3.6GHz
Display:ASUS VW228 1920x1080
RAM:8130 MB
MotherBoard:ASUS H81M-A
Power Supply:MAXPRO 600W

It could sound strange but try changing the frecuency the image is displayed ... some monitors supports exclusively 24/50/60 hz. So i would reccomend you to enter the nvidia panel in the resolution settings custom resolution create a 50 hz one. And try any game you want...
