Poor overclock results - Am I missing something?

Hey Guys,

I came to this forum because the information here is what made me trying to overclock my rig.
Specs:

be quiet! Pure Power CM L8 730W voeding
Intel Core i5-4670K (4x 3400 MHz) processor (Haswell, socket 1150)
Cooler Master Hyper 412S CPU cooler
Gigabyte GA-Z87MX-D3H (usb 3.0, sound, lan, s-ata600)
Kingston HyperX 8 GB (DDR3/1600MHz/CL9) (1x8 GB)
MSI N770 TwinFrozr 2GD5/OC (NVIDIA GeForce GTX 770, 2 GB)

I put my voltage at around 1.22
My Mhz at 4.1
Disabled turbo
Disabled C-states

Even played around with my uncore & Ring V. But it all makes no sense. crashes all the time!
4.1 was kinda stable but after 4 minutes of stress testing, my pc frooze. Went down to 4.0.. still the same. I saw people getting 4.5+ with a comparable system.

Am i missing something or am I unlocky with a bad chip?

I think you just got bad chip, i have 4670k too and all i did was sync all cores, multiplier and core voltage, i got bad too, 4.3GHz with 1.22v cant get 4.4ghz until 1.3v
