My cousin runs a machine shop and we are considering doing some
fabrication of replacement parts on the side. What are the heavy
hitters?

Pacman joystick centering grommets, joystick shafts, test switch
brackets.

I think the most in-demand parts would be reproduction parts for Coin
Controls coin doors.

I bet there would be a few folks that would like a new Major Havok
Track Roller. I know someone did this a few years ago but even those
are long gone.

Joystick centering grommets are still available, just ask your
distributor for NFL Blitz joystick grommets, they're the same thing.
Or save the hassle and just order from Arcadeshop.

Yeah, the rollers with the 'K' are hard to find.

Would also work for the Atari game Accelerator.

I've used both and unless my distributor had their bins mixed up, Blitz
grommets fit perfectly and had the same feel.

Atari Hall Effect sticks are scarce (I, Robot, Escape from the Planet
of the Robot Monsters, Road Runner, etc). Last few I saw sell on eBay
went for well over $100 each.

Kelsey at Oscar Controls has stated that the Major Havoc roller
controller is on his "to do" list. Last time I exchanged emails with
him was a couple of months ago and the roller controller was still on
his list.

I thought Blitz grommets were a bit smaller than Pac ones. I know the
Arcadeshop ones are dead on.

Yeah, but who needs hall-effect sticks when you can replace them with
49-way sticks :)
