Question Random loss of screen signal vega 56 gigabyte

Got a "renewed" gigabyte vega 56 oc from Amazon to put in my first build I finished two days ago. I have a r5 2600 and a 650w gold psu. My computer randomly (more often when under load like games) loses screen signal, but internal fans and LEDs stay on. One odd thing happened right before I got off my PC tonight; the normal screen issues happened, but the GPU fans started going at 100% speed. My CPU is not overclocked, and my GPU is set to 1650 core frequency and 900 memory frequency (mhz). I have these set through afterburner, and have a power cap of +20%. Any thoughts on my issue? I also have strange frequencies when I play, like sub 1000 mhz core clock, when the card is set for a base of 1100, and a memory clock of 700 or 900, one of the two. Any help?

My only thoughts sp far are that the card is messed up, or the psu is insufficient.
