Replacing heatsink fan on HP H8-1214

I have an HP H8-1214 (I know it's not great...bought it from my friend for cheap after my last computer bit the dust, but I'm saving for an upcoming build in a few months) and the heatsink fan has gotten quite squeaky/chirpy even when the CPU is at idle load, so I think the bearings are going bad. I'd like to replace just the fan, but I'm a little confused.

The screws connecting the fan enclosure to the heatsink are driven through the bottom set of holes of the fan enclosure. I'm not a crafty person so this may be a dumb question, but are there screwdrivers with shafts small enough to fit through the top set of holes so that I can reach the bottom set to unscrew the screws and remove just the fan part (and then replace it with another one)? Or do I need to remove and replace the whole thing with another combined heatsink/fan unit? If I do need to replace the whole thing, how do I do it safely?
