RX580 black screens during Witcher 3

I just got myself a new graphics card the XFX Radeon RX 580 GTS XXX Edition 8GB
When I'm not ingame there are no problems at all but when I play like 2 mins of the witcher 3 on ultra black screens appear. They take 1-3 seconds and I can still hear the game in the background and after it comes back the game is still running but it keeps doing this every like 5-10 second. I don't think it is overheating because the temperature is around 81°C and the fans work at 65%
Here is my setup:
Seasonic S12II-620 620W Bronze
ASRock B85M-HDS
i5 4440
XFX Radeon RX 580 GTS XXX Edition 8GB
8GB DDR3 1600Mhz
I also have an old HD LG monitor which still has a VGA input and I need to use a VGA TO HMDI transformator because the card doesn't have an input like that.

did you delete old gpu drivers before dropping the 580?
Use ddu cleaner to clean the gpu driver and install new one from XFX website.

I did it but it still happens.
If I noticed right it happens when my GPU reaches 80°C so it might be overheating problem or idk.

Thanks man I set a custom fan curve and it is nice and steady at 65-70°c
