I'm honestly stumped on this. I've been having this issue for quite a while, and after changing my mouse, another problem I had which was the reboot and select proper boot device problem went away. At first, I thought it might have been my keyboard, but I have unplugged it and tried but it still persists. I cannot capture it using shadowplay/FRAPS and it seems that the severity of the tear is completely random, as I have tried it again and with other applications and it changes on where it tears.
I'm running on a Windows 7 Home Premium, Intel i7-3820, NVidia GeForce GTX 680, and 8 gigs of RAM. This is my monitor:
