Streaming via game capture card

Greetings. I am watching some twitch streamers and they have crystal clear full HD picture. I have good computer, 12MB upload and when I setup my bitrate to more than 1500kbps people cannot watch the stream..

Which means I cannot stream with 1080p. Is this possible only for twitch partners?

Or do they use game capture card to somehow provide crystal clear picture to twitch? How does that work?
