Sub 2 MB/s disk usage, already maxing out at 100%

So for the last few weeks, my PC has been running painfully slow. I noticed my disk in the task manager was maxing out for no reason, so I decided to do the old reset, and after that, plus removing all personal data, it stills maxes out at around 2 MB/s usage, but sometimes up to 3 MB/s. I find this weird, since previously it could run multiple instances of Firefox and Sony Vegas at the same time, but now I can barley have a browser open, and this is making me worried.

My specs are the following;
GTX 960 4GB GPU
i5 4460 CPU
An unknown MOBO
8GB DDR3 RAM
And a 1TB drive that should be able to handle up to 5 GB/s speeds.

100% disk usage is usually a clear sign that the HDD is dying. I suggest you replace your HDD.

I had the same thing happen on my AMD build a few years ago. My disk usage went also 100% and after some weeks, using the HDD got even worse. I regularly do disk defragmentation on my HDDs and when the drive was new, it took about 2h per drive with 70% of the drive being full. But at the end of the drive's life, disk defragmentation took 58h on that drive to complete. Ended up RMA my HDD (since it was 1 year old) and i got a replacement drive that is currently in use in my Skylake build.
