There are 3 options:

Normal
Diagnostic
Selective

What do you use and why?

I have it on "selective". Then under"startup" I have it only open my anti virus program and windows defender.

All of the msconfig defaults should only be changed when troubleshooting a problem, otherwise it should be left alone.

If you want to permanently disable one or more startup items you should instead use a third-party utility such as "Ccleaner": http://www.filehippo.com/download_ccleaner/
