Unable to converting my 1tb internal harddrive from RAW format to NTFS format.

I have been attempting to reformat a RAW harddrive to NTFS so that it is usable, but i keep getting an I/O error of some sort. see screenshots below:

Has anybody else ever run into this? Is my Hard drive not usable?

Try a new data cable to the problem drive.
