Video Card not being detected by Windows/MB

Hello,

I recently built my own PC and I have been having issues where the graphics card is sometimes not detected by the motherboard on boot. This happens 50% of the time and the other 50% the intel gpu is used. When it is not being detected the fan still spins, which I guess means it is getting power? is this a problem with my MB or GPU?

UPDATE: I am plugging it into the GPU, but when computer is on and I am getting no signal I plug it into the motherboard and the intel gpu has been loaded up.

MB: gigabyte gaming 5
Graphics: gtx 960
Screen: Pavilion 23xi

are you plugging the monitor into the graphics card? not the motherboard?

Try removing the graphics card and reseating it.
