What is going on with the render distance when I play?

I've noticed for when I've been playing games over the past few months, I've been having issues of models / textures only loading when I've been close or not loading properly at all.
In Skyrim treeps are popping in everywhere constantly
State of decay models like boxes are only rendering when I'm close and are just randomly disappearing
In h1z1 the floor / ground in general doesn't load like properly. (its very low res and sometimes the ground is like above my charachter and I am in the floor and can't see my self or anyone)
Is my cpu or hdd having problems loading stuff? Do I need to defrag my hard drive? What is going on?
 
Solution
when you update the driver, select custom and check the box where it says "clean install"

Since you are on newest drivers.

Install Advanced System Care (free)

Uninstall Nvidia Drivers.

Run Advanced system care to clean all drivers and registry.

ReInstall all latest NVidia drivers.

Edit:
Also go to Device Manager,

Expand everything,

Right Click on Everything(each thing, long and annoying process) and Update using Online Drivers (from Microsoft)

I don't think I've updated it since I got the card in October. So would this be the issue?
