What the hell is happening with my USB's???

Okay so every now and then, everything plugged in via usb "stops working" I hear the windows sound that something unplugged....and then moments later they come back on....I mean its not major but as a gamer then it becomes major??? Help? Does anyone know what might be causing this and or how to fix it??

either driver or hardware issue.

dl newest driver from board/chip manufacturer site, and ccleaner
unplug all stuff not needed.
uninstall usb driver (chipset if possible), reboot
clean with ccleaner incl reg cleaner, reboot
install chipset driver, reboot
install, usb driver, reboot
