Where can I get mic/spk ports for my front panel?

My spk port broke on my front panel, so I took it out and it came on the same plate as the mic port. The 2 USB ports are on a different plate. So I was wondering where I could get a new plate with just a spk and mic port on it for my front panel. I have a Phanteks Eclipse P400 case.
