Which graphics card is perfect for my PC? Please answer carefully.

My PCs specs:
Intel pentium g630 2.7 ghz
Intel dh61ho motherboard (2nd generation)
400w ATX switching power supply (non brand)
4gb ram
Current gpu: GeForce 210

Given your system specs, I'd like to ask what you do with this PC before offering a suggestion.

Your system is really limiting and you would get the best bang for your buck by building an entirely new system, but for an upgrade I'd say that a secondhand gtx 750ti would be your best bet. If you don't want to buy secondhand, then just get an rx 460, your cpu will likely bottleneck it but the price is the same as a new gtx 750ti so there is no reason not too.

i would like to play games like gta 5, wicther 3, assasins creed unity, assassins creed syndicate on low-medium resoulotion.
