No, not well enough to be playable. It was a good card in its day but it's not up to modern games.

No it won't Fallout 4 needs a DirectX 11 capable graphics card to even launch. The 9800GTX+ is only DirectX 10 capable. You need a new GPU at the very least, and possibly more upgrades if the rest of your system is as old as that graphics card.
