Question Windows Hardware Error

Hi been having this problem a while, whenever i launch the game "heartstones" and rarely other games my monitors go black and some of my programs crash. (pc doesnt shut down)
i get a "reliability monitor" saying Windows Hardware error

Edit: i have ran memtest86 test came back all fine (RAM ok)
Psu is new just last year was replaced, not drawing too much power
