Windows won't boot correctly

About a week ago my computer started to slow down and I noticed that my HDD disk usage was at 100%. I tried to restart my PC and it wouldn't get past the bios flash screen. I was able to reinstall my OS and my computer was working fine again. Now however I am having the same problem. Startup repair won't work neither will ,sfc/scannow, windows refresh, startup repair,or rebuild BCD. Now I have error code 0xc00000e9. Could it be my hardrive needs to be replaced?

use the hdd maker software and test it also save all the data you could need when you have acces to it after test .

This is the the problem with your hard drive. so you need to check it properly.
