What would be the best NVIDIA card for running Skyrim and Fallout: NV at high setting at around 40 FPS? I would prefer the card to be under $300, but if there is one for around $330 or less, let me know. I won't be using any graphics mods. My build is below,,,

Intel i5 4690K
Cooler Master Hyper 212 EVO
Asus Z97-A
Corsair Vengeance 8GB DDR3 1600
Samsung 840 EVO 120GB
Western Digital 1TB
Cooler Master V650 PSU
Fractal Design Define R4
Asus VE228H 1080P Monitor
Windows 8.1

Solution
I'd really recommend stretching budget a little and 970 instead, unless you are using the 760 as an interim card until the next gen, your build is very CPU heavy.
I have your CPU in my main PC and have tested it with a GTX 750 Ti (which is now in my second PC with an old Phenom II). It easily hit 60 FPS everywhere in Skyrim (stock - no graphics mods) on the "High" preset. It could do Ultra @ 45-60 FPS as long as I turned the anti-aliasing down to 4X. This is on a 1920x1200 monitor.

Try to get a GTX 970.
Some of them are under 330$ (Disclaimer: atleast where i live and shop for parts)
If you can, wait till after christmas, prices tend to drop a bit.

I'm telling you to get a GTX 970 for 2 reasons,
1- You have only a 650w power supply, so you want to go Nvidia cards cause they have lower power consumption.
2- The 970 is pretty much the best combination of Power Efficiency and performance.

EDIT: Found a GTX 970 for 330$

Thanks! I was really debating whether to get a 750ti or a 760 and the 760 won me over. Would I need to get more VRAM if I have 30+ mods installed or would a 2GB GTX 760 work well?

No graphics mods, mods like Immersive Armors and Weapons, a few player homes, one or two overhauls and immersion mods. I personally think Skyrim look great on vanilla.

All the Wet and Cold mods, Realistic Needs and Diseases, a few houses, two quest mods, a thievery overhaul, Immersive First Person View, Random Alternate Start and Character Creation Overhaul, Enchanted Arsenal, and Immersive Armors and Weapons.

Then a 760 will work.
