You forgot:
Itemization - spells locked to weapon dmg and strength rolling on Wizard items, etc.
Area randomization - the replayability is in the pits because there is virtually zero level randomization.
Story - D1, D2 story was made by adults, for adults. D3 story was made by WoW children for children.
There were, of course, a lot of other smaller mistakes. Way too many crap rare drops (which they're finally fixing iirc in the 1.0.8 patch, it's about darn time)
Paragon levels - despite their popularity, I believe implementing this was a mistake.
Always online, on the other hand, was completely necessary, unfortunately, and the right move. It's also unfortunate that they were unable to completely prevent botters & hackers.
