BSOD Issue all of a sudden when playing games

Recently I a day or two after getting witcher 3 along with the new driver update i have been getting BSOD that are related to the graphics card i beilive or the graphics drivers. It happens on most games i play including skyrim Witcher 3 and CSGO. I have a GTX 970

Specs : MSI GTX 970 , I5 4690k , ASRock H97m Pro4 , corsair 750m

I would say one of the most common causes of BSOD is Faulty Memory, just something to keep in mind if your new PSU still has issues. Maybe even a faulty memory slot, try moving the memory to different slots. Maybe your PCI-E port is shot, maybe your GPU is shot, do you have any other PCI devices installed? Take them out. In fact just strip the computer down to the most bare bones as you can then add 1 thing back at a time until it BSOD.
Just a few ideas.

1) try system restore.
Or
2) Delete all the files in 'temp' folder. Then once again install the updated drivers of graphic card

3) if you are using windows...., do "driver verifier". By doing this.., it will be easy to identify , which driver is causing BSOD.

My recent BSOD problem was the motherboard- I went through everything else mentioned here as well as other stuff before reaching the answer. Looking closely at the m/b there was some bulging of the capacitors so that was probably the cause- capacitor have a finite life and can fail.
But go through the other things first before deciding that's the problem- the advice you've been given is good if a little unstructured. I'd add-
Have you checked your CPU temperature as it fails? It might be worth reseating the processor- clean the old thermal stuff away thoroughly and use some new compound then carefully reseat the processor.

I don't know the age of your machine but I've seen this happen when the CPU/fan was thick with collected dust.

Do you have a friend with a similar computer who'd be willing to help? Maybe difficult to arrange but may be worth the effort. Get the 2 computers together then try a bit of swapping parts and testing to see if the problem goes.Try his bits in your computer and try your bits in their pc to see if that causes problems to go or to move to the second machine. Try the memory, Power Supply etc. one thing at a time replacing each item after testing. To save time only test your machine until you find it doesn't have the problem.

Does the problem occur if you're not gaming? Does it happen on other games too?

What model Power Supply do you have? What are it's rated outputs?
I assume that the Voltage reading you gave was when the machine was idle- you need to check that voltage under full load because it could drop significantly. Monitor he voltage pre-game, while it's running, and as the computer BSOD,s. Voltage drop under could be your problem.
