Computer Black Screen crashing randomly - GPU / PSU Problem?

Hey, recently I bought a new monitor (1440p 75Hz, from my normal 1080p 60Hz), and a few days after I've been experiencing Black Screen crashes, which require me to manually turn off my computer.

Initially, I thought it a problem with my new monitor; I thought perhaps the DisplayPort cable was faulty and sending an unwanted charge which was causing problems, but it continued with HDMI, and I have recently tried using my old monitor, and the problem has persisted. Sometimes my computer goes days without the issue, but others it occurs more than once a day (such as today).

I formatted my SSD and reinstalled Windows, in case there happened to be any malware causing problems, but the problem still occurs. After removing my GPU, the problem does not occur. So, I suspect that the problem is to do with my GPU, Motherboard, or PSU.

I am using an AMD Sapphire R9 390 8GB. Here are some logs I took to help with the problem (I think the CPU-Z report is without my GPU plugged in, as my PC crashed too frequently to install programs). The GPU-Z log shows before and after a crash - note that my GPU load, despite not performing any intensive tasks, spikes to 100%, as well as my voltage rising in the last second before the crash.

I asked for some help on Reddit and they advised I run a CPU and GPU stress test and log with HWiNFO,m so that log has been attached too. No one seemed to be able to decide if this was leaning more towards a PSU problem or not, so I thought I'd get some second opinions here.

TL;DR:

  • Black screen crash, must forcibly restart
    High GPU load 1 second before crash, & increased voltage
    Problem eliminated by removing GPU (AMD Sapphire Radeon R9 390 8GB)
    Potential PSU Problem (EVGA SuperNova 750G)
    Check CPU-Z and GPU-Z logs

The only Critical messages are from when I have to forcibly power off the machine as it is frozen on a black screen . Upon freezing there is no sound at all, all sounds that was being played immediately cuts off during the crash.
