Bit of a weird one...
My cpu benchmarks low on multi thread when playing games such as fortnite, apex warzone... its multi thread benchmarking at 700 but when I restart the pc and don’t launch a game it’s at 5700.. I use rog cpu-z to check this

this is a strange problem as if I delete the affected game and reinstall the problem doesn’t happen anymore for a week or so then comes back and I have to repeat the delete and reinstall

I have updated chipset, graphics drivers etc
Reinstalled windows and started again
Problems still here

specs
I9-9900k
ROG Hero Maximus motherboard
GeForce rtx 3080 suprim x
32gb Corsair ram (2x16)
Water cooled

(cpu temps are never over 27)

any help would be much appreciated

If your temps are at only 30-35C during, I'd guess you are at 800 MHz? (Check clock speeds sustained via HWMonitor under a load induced by CPU-Z/bench/stress cpu

If only at 800 MHz, this often happens before chipset drivers are installed, or with same botched by a Win10 update, a glitch, etc...)

Running a benchmark while also playing a game WILL result in horrible bench scores. That's like trying to mow the lawn while running timed laps at the track and worried why your times are so bad.
I understand that but when I’m doing it when everything’s normal I’ll still get 3000 on the bench when it’s benching at 700 I’m getting less than 20 FPS but if it’s at the normal 3000 with games launched I’ll get 180+
